As a policyholder of retirement benefits from MetLife Assurance, you may need to use the forms provided below.
Your pension payment is based on information received from the trustees of your Occupational Pension Scheme at the time your benefits were secured, and any changes that we have been informed of subsequently.
If the details shown in your Summary Benefit Statement (found in your Welcome Pack) are incomplete or inaccurate, or your personal circumstances have since changed, please inform us as soon as possible.
Attached is a form you should use to inform us of a change in your personal details. It is important that you return this form promptly, as your next pension payment may be delayed or interrupted if your details are incomplete.
